Our main aim is to support the young people in our care to ensure that they are prepared for a life of independence with basic life skills and they get help and support with any issues that they are facing.
Our services have been designed to be a bridging gap that offers young people the experience of living independently with support as when they require it.
We have fully trained support workers who have vast experience with working with young people and assisting them with any issues they may face as they transition to independent living and adulthood.
How It Works
Every young person is appointed their own key worker who acts as their point of contact with their everyday needs. Together with their social workers and PA’s, the key workers will draft a support plan where the young person will input their needs, goals and expected outcomes.
This will form the pathway plan that the young person together with everyone involved will work towards to achieve their outcomes.
Their will be regular keywork sessions with the keyworkers where the plan is reviewed. This provides an opportunity to discuss any issues and the plan as amended as required.
Our main focus is ensuring the young person achieves their goals and outcomes. We will therefore work with them to ensure that we are constantly meeting their needs and their achieve their outcomes.
